What to Expect:

24 hours prior to your scheduled appointment, we will verify that your insurance coverage is active.
After confirmation of active health insurance, one of our healthcare navigators will call, email and/or text confirmation of your scheduled appointment the day before your appointment.
On the day of your appointment, we will call, email and/or text to let you know that we are on our way to you – wherever you call home.
We will complete an in-home assessment of your health status to include a review of your medications and of your surroundings to ensure we minimize any potential fall hazards.
Based on your current health status we will determine if you qualify for certain Medicare-approved programs.
We will determine what your needs and goals are and collaborate with you and your caregiver working together to listen, support, and create the best care plan for you.
